Intra-Articular Bupivacaine-Morphine for pain Relief after Knee Arthroscopy A Randomized, Prospective, Doube-blind Study

Abstract
We performed a randomized, prospective, double-blind study in patients undergoing elective knee arthroscopy to assess the effect of intraarticular bupivacaine and morphine on postoperative pain.
Sixty ASA physical status 1-2 patients, having knee arthroscopy, were randomized into one of three groups. All patient received general anesthesia with N2O, O2, and enflurane. At the end of the surgery, the following were injected intraarticulary
:Goup
1(n=20), 0.25% bupivacaine : Goup 2 : (n=200, 2mg morphine in saline ; Group 3 : (n=20), 2mg mrphine in 0.25% bupivacaine. The volume injected was 25 ml and all solutions contained 1:100 epinephrine 0.1 mg.
Postoperative pain was assessed over the ensuing 24 hours at intervals by a visual analogue scale and the requirement of supplementary analgesia was recorded.
Our results indicted that there was no significant difference in pain scores, consumption of additional analgesia, or time to first request for analgesia between any of groups. We concluded that intraarticular injection of bupivacaine after
arthroscopic
knee surgery provided prolonged analgesia but there is no sutgnificant prolonged analgesia provided by intraartricular morphine.
